If so, please complete the Workshop Proposal form.  You must use this form to submit your proposal for a workshop (one form per workshop) to be presented in Lily Dale, NY, during the next summer program season.  Lily Dale Assembly must receive submissions no later than September 1st to be considered for the following summer season.

Only email proposals will be accepted and must be accompanied by a professionally prepared jpeg photo.
